Hospice at Home
Our Hospice at Home team gives patients, their families and carers, access to high quality, compassionate care in the place they most want to be... at home or in their care home.
Led by Dr Cate Seton-Jones, the team comprises consultants, specialty doctors, GP trainees and junior doctors in training. The consultant staff are all specialists in palliative medicine and bring a wide range of experience from both general practice and hospital medicine backgrounds.
They are supported by specialty doctors and GP clinical trainees, all of whom have years of experience.
As well as caring for patients, our medics also train junior doctors and medical students, lead training and education courses for other healthcare professionals, and help to develop local palliative care services for patients, monitoring and improving standards of care to ensure patient safety and the delivery of the best quality services possible.
Our Nursing team comprises clinical nurse specialists (CNSs), registered nurses and health care assistants. They provide high quality palliative care, help patients to maximise their independence and improve their quality of life, and offer emotional support to patients and their families.
Our CNS team is often the first point of contact patients have with us after their referral. They assess patients and families; provide expert advice, advance care planning and end of life care; offer emotional, psychological and practical support for patients and carers; liaise between patients, GPs and district nurses; and refer patients and relatives on to other services within Phyllis Tuckwell, as required. Our Hospice at Home nursing team is led by registered nurses with specialist palliative care experience and supported by health care assistants. They work closely with GPs, district nurses, and community and hospital palliative care teams, as well as with other healthcare professionals. With their help, patients are able to spend their last days at home or in their care home, instead of in hospital or on our In-Patient Unit.
Our In-Patient Unit is run by a ward manager, and consists of sisters, registered nurses and health care assistants. They treat each patient as an individual, offering personalised care and support, and ensuring that their privacy and dignity is maintained at all times.
Our Living Well service is supported by doctors, sisters, registered nurses and health care assistants, who support patients in their own homes or care homes, and in sessions at the Beacon Centre.

Our In-Patient Unit (IPU) is currently located in Camberley where we have 10 single en-suite rooms. Our passion is to support you and your family, offering comfort and understanding at this difficult time - as well as at times of hope and joy
Our Living Well service is designed to help patients living with an advanced or terminal illness, and those closest to them, to manage the impact of their illness, cope with changes, improve wellbeing and remain as independent as possible.
All our services are free, but this is only possible due to the generosity of our amazing community. The Government/NHS only provide 25% of our costs, so we need to raise over £25,000 every day to provide our vital services.
