Hospice Care at Home

Our Community team gives patients, their families and carers, access to high quality, compassionate care in the place they most want to be... at home or in their care home.

You'll receive specialist palliative care

Our services bring the skills and expertise of our specialist palliatve consultants and doctors, nurses, health care assistants, patient and family advisors, Pastoral Care team, therapists, benefits advisors and counsellors straight into your home. They will assess your needs, offer advice on managing complex systems, assist with advance care planning, provide physical and psychological care, provide access to information and therapies tailored to your needs, and those of your family or carer, and also support you at the end of life.

We also work with other healthcare professionals to support rapid discharges from hospital and help reduce unplanned hospital admissions.

The Community team works closely with GPs, district nurses and other hospital healthcare professionals involved in your care providing nursing and personal care, helping you and your family cope with a terminal diagnosis and the final few weeks and days of life at home. The team supports the District Nursing team, who hold the central role in the delivery of care in the last days of life for our patients at home, with specialist support, care and advice.

Volunteer-led home support

To enable patients to stay at home, sometimes for much longer than would otherwise be possible, we also provide volunteer support to patients and their families in their own homes through our Home Support (HS) service. 

This could include assistance with shopping, ironing, taking the patient on outings, helping with their children, accompanying the patient on hospital visits or simply allowing the carer a few hours break. An HS volunteer is often described as a "good friend" by many of the HS patients - someone who will listen and befriend. 

The HS volunteers work closely with Phyllis Tuckwell's clinical nurse specialists and Patient & Family Support team to provide a seamless service, dedicated to supporting patients and their families at an extremely difficult point in their lives.

All our services are free, but this is only possible due to the generosity of our amazing community. The Government/NHS only provide 25% of our costs, so we need to raise over £25,000 every day to provide our vital services.
