Nursing in the last days
Our Hospice Care at Home team provides compassionate nursing care for patients who have chosen to spend their last days in the comfort and familiarity of their own homes.
District nurses and GPs hold a central role in the delivery of care in the last days of life for our patients at home and we support them with specialist support, care and advice.
Our team is made up of registered nurses and health care assistants, all of whom look after patients in the last few weeks of their life, visiting them, getting to know them and ensuring that their last wishes are heard and respected.
They support patients’ families too, helping them through the emotional and practical difficulties which they may be facing. With their help, patients are able to spend their last precious days where they want to be – at home, surrounded by their loved ones and their memories.
