Phyllis Tuckwell Hospice Care provides supportive and end of life care for people living with an advanced or terminal illness. Based in Farnham and Guildford, we serve a population of 550,000 across West Surrey and North East Hampshire.

 Hospice Care at Home Nurse Team Lead

 28.5 hours per week (e.g. 3 x 9.5 hr days)

Salary: £28,381.44 (based on a WTE salary of £ 37,344.00)

 

We are seeking to recruit an experienced registered nurse with community palliative and end of life care experience and excellent communication skills, to join our Hospice Care at Home team as a Team Lead. Current clinical skills are essential to support responsive clinical care in the home setting, as are leadership, management and organisational skills.

Based primarily at our Farnham site, with travel to the Beacon Centre in Guildford and into the community, you will work across the week to provide clinical leadership to the Hospice Care at Home team who provide direct nursing and personal care at home for patients in the last weeks of life. You will have a defined team that you will have direct line management responsibility for, and this will include appraisals and supporting staff development. The clinical part of this role will involve direct patient care with your team, plus rotation into the co-ordination role where you will be receiving referrals, clinically prioritising and liaising with community nurses, patients, and carers to deliver a high quality and responsive service.
