Advanced Nurse Practitioner

Hours available: 37.5hrs per week (WTE) or 30hrs per week (0.8 WTE) dependent on expert skills, qualifications and experience.

Base: The Beacon Centre in Guildford with the expectation of working across our geographical area and PTHC sites.

We are seeking to recruit an experienced senior nursing clinician with an MSc in Advanced Clinical Practice, and preferably the V300 Non Medical Prescribing qualification, senior nursing leadership experience, and a minimum of 5 years in current palliative & end of life care clinical practice, to work across our geographical area of Guildford, Waverley, Surrey Heath, NE Hampshire and Farnham with our Care at Home and Living Well teams.

Current expertise in community palliative care and commitment to team working is essential, with the post holder assessing patients with complexity, symptom burden, supporting advance care planning and clinical practice. The scope of the role will be in the Patient’s home, PTHC outpatient and Living Well settings, across the 7 day week, taking part in the rapid response clinical rota during the day, and out of hours on call Lead rota alongside other Leads.

A focus on quality improvement, clinical audit, learning from practice, and education is integral to the role.
