Inpatient Referrals

Phyllis Tuckwell has a 10-bed In-Patient Unit (IPU) at Kings Lodge care home in Camberley.
Patients may be admitted for:

  • General admissions: symptom management, palliative rehabilitation, palliative interventions, and end of life care
  • Emergency respite: eg when a carer can no longer maintain their carer role, and is seeking a short term solution whilst future plans are made.
  • Post-event: e.g. following radiotherapy for Spinal Cord Compression or following RIG insertion.
  • Interventions: e.g. blood transfusions, bisphosphonates, straightforward paracentesis

Patients may be admitted from the PT catchment area and Midhurst Macmillan Palliative Care Services. Out of area referrals are considered on an individual basis.

Referral
Referrals to the In-Patient Unit may be for patients known to the Phyllis Tuckwell team or new referrals. If the referral is new, the local specialist palliative care referral form should be completed.

If the request is urgent, the referrer should phone and ask to speak to our ART team about admissions.

Contact our Advice and Referral Team (ART) of clinicians and administrators. HCPs can contact them directly to ask for advice about a patient with an advanced or terminal illness, or refer them to the Phyllis Tuckwell service.

The ART team are available 7 days a week, from 8am – 6pm.

Out of hours, calls will be transferred to our In-Patient Unit.

Referrals can also be made using our downloadable referral form which needs to be completed by the patient’s GP, hospital doctor, clinical nurse specialist or district nurse and returned to us. Please note, the referral form consists of two pages which will need to be fully completed. Once completed please email [email protected]
