Care at Home Referrals

The aim of this service is to enable choice for patients to remain at home, if this is their preference, and to respond to clinical deterioration in times of need.

The Care at Home team, consisting of our multi-disciplinary specialists, includes our Hospice Care at Home (HCAH) nursing team, who provide practical palliative nursing for patients who have symptomatic needs at the end of their life, and also provide support for their families.

Referral
Referral may be considered for patients who are appropriate for specialist palliative care and:

  • Wish to be at home for as long as possible
  • Who require end of life nursing and support
  • Have unstable symptoms

The service may also be appropriate for patients who clinically deteriorate rapidly or who experience a psychosocial crisis and need additional support to stay at home, even if life expectancy is thought to be longer than a few weeks.

Patients may already be known to the Phyllis Tuckwell team.

Contact our Advice & Referral Team (ART) of clinicians and administrators. HCPs can contact them directly to ask for advice about a patient with an advanced or terminal illness, or refer them to the Phyllis Tuckwell service.

The ART team are available 7 days a week, from 8am – 6pm.

Out of hours, calls will be transferred to our In-Patient Unit.

Referrals can also be made using our downloadable referral form which needs to be completed by the patient’s GP, Hospital Doctor, Clinical Nurse Specialist or District Nurse and returned to us. Please note, the referral form consists of two pages which will need to be fully completed. Once completed please email [email protected]
