The Hospice Care at Home Nurses provide timely practical, skilled care in the patient’s home offering calm, sensitive emotional support to the patients and their families.  

The Hospice Care at Home Team works closely with our Community Specialist Team. Their diligent care, quality choice, symptom relief and continuity enables patients to remain at home who may have only a short time to live if this is their choice.

We find that we are often the patient’s advocate and carer voice at a difficult time of life when psychological need may be very high. The service is on offer to all patients registered with the Hospice. All our Nurses work alongside the GP and Community Nursing Teams including Macmillan and Marie Curie Cancer Care.
